The Centers for Medicare & Medicaid Services (CMS) discovered an error in measure calculations for the Discharge to Community – Post Acute Care (PAC) Skilled Nursing Facility Quality Reporting Program measure and are re-releasing the corrected measure data in a refresh scheduled for September 29, 2021. The data are based on Medicare claims data submitted to CMS for the FY2018-2019 reporting period (01/01/2017-09/30/2019).
