JOB OVERVIEW:
Performs services that enhance the psychosocial, emotional and intellectual development of residents and family members with personal and environmental difficulties.
JOB DESCRIPTION:
• Prepares resident assessments, gathers social history, develops resident care plans as they relate to psychosocial needs and issues. Documents review and updates records and care plans.
• Facilitates residents and families in planning objectives and goals for the residents.
• Provides crisis intervention, ongoing support, assists with problem solving and support of residents and families. Assists in obtaining resources from community social, health and welfare agencies.
• Provides consultation to members of the staff and community agencies in support of residents and families.
• Interprets social, psychological and emotional needs of the residents and families to the medical staff, attending physician and other resident care team members.
• Participates with Interdisciplinary Team meetings and other facility committees.
• Participates in discharge planning and referrals to community agencies.
• Coordinates with mental health consultants.
• Consults and provides residents and family members information regarding Medicare, Medicaid and available financial assistance programs available to the resident.
• Advocates for and helps protect the rights of residents.
• Assists in review and follow up of complaints concerning resident’s lost items, resident/family grievances in accordance with established company procedures.
• Remain current on all applicable federal, state, and local laws, regulations and professional standards.
PREREQUISITES:
• Bachelor’s Degree
• Willingness to provide compassionate and professional care to residents.
• Ability to communicate in English, both verbally and in writing.
• Ability to write clearly, concisely and accurately.
• Skilled in interpersonal relationships.
• Ability and willingness to work with a diverse population.
• Superior judgment skills.
• Ability to multi task.
