RN Resident Care Manager / MDS Coordinator

Rose Villa, an independent, non-profit Continuing Care Retirement Community, is seeking a Resident Care Manager (RCM) for Madrona Grove Supportive Living, which is a licensed nursing facility (ICF level – no skilled nursing, which means no PPS MDS schedule). This is a 40-hour per week, salaried position with a maximum case load of 22 residents.

Our RCM must have excellent nursing skills and a passion for serving elders, particularly those with dementia. A high value must be fostering autonomy and choice, no matter what a person’s physical or cognitive status may be. Our RCM must hold a valid Oregon RN license and be able to pass a drug screen as well as a criminal background check.

The RCM is responsible for leading the care team through the Minimum Data Set (MDS) process and ensuring timely and accurate submission to CMS. MDS experience is preferred but not required. A significant part of this process is that of guiding the care team to create and implement comprehensive, person-centered care plans that build on resident strengths, supporting residents to live the life they want to live. The RCM also leads quarterly care conferences with staff, resident, and resident families and is the primary point of contact for family members with questions or suggestions regarding their loved one’s care.

This is a hands-on position with a lot of resident, staff, and resident family interaction. Excellent interpersonal and communication skills are a must. The person in this position must also be very comfortable navigating an electronic health record and working in Word, Excel, and Outlook. Attention to detail is critical.

We are a small community and, as such, teamwork is highly valued. This means that every licensed staff person will occasionally need to work on the floor when the need arises. This position is one of two RCMs that are part of that team, as is the Director of Nursing. The RCM may also be the Acting Director of Nursing when the Director of Nursing is unavailable.

The RCM generally works during normal business hours but should flex their schedule in order to have a connection with and gain input from all three shifts (ex: coming in at 6:30am or 10:30pm once or twice a week to connect with night shift). The RCM is also part of the Manager on Duty rotation and will work approximately seven weekends per year, taking days of during the week to compensate.

This is a full-time exempt position. Salary is competitive and commensurate with experience.
