Hospital Discharge Incentives Extended
DSHS has again received approval to extend the admission incentive being offered to SNF/ALF. For SNFs, the admission incentive continues at $6000 for each newly admitted resident from an acute care hospital. Assisted living facilities continue the tiered incentive:
| Admissions Incentive Period: | Through June 30, 2022 |
| Admission Incentive: $ 3000.00 | 59 days or less since HCS referral |
| Admission Incentive: $ 6000.00 | 60 days or greater since HCS referral |
The incentives to SNF/ALF providers cover newly admitting individuals from acute hospitals that occur through June 30, 2022.
