JOB SUMMARY

The Wellness Nurse is responsible for the health care and related services available to independent and Catered Living residents through WHCHS. Promoting the safety and well-being of the independent and catered living residents is primary for the Wellness Nurse and this is done through nursing assessment, early intervention, and health education. This position plays a key role in educating all residents regarding supportive home services available on campus.

ESSENTIAL FUNCTIONS

Functions as a health resource for campus residents,
Provides health related education, answers questions, contacts physician on behalf of a resident,
Performs health screenings and scheduled clinic sessions as assigned,
Responds to emergency calls during business hours/ scheduled work hours.
Performs the functions of a Registered Nurse as defined by the Nurse Practice Act of WA State.
Completes resident health screening at intake and on-going as assigned,
Completes reassessments at defined intervals,
Completes licensed skilled nursing care under the home health license, in compliance with applicable rules and regulation, and organization policy and procedure.
Supervises LPN as appropriate; provides oversight of quality of care provided by Homecare and campus staff as it relates to safety, patient safety, and customer satisfaction.
Demonstrates proficiency in technology.
Completes documentation and data entry into the EMR according to policy and within required timeframes,
Client clinical records and other documentation is complete at the end of each shift.
Completes documentation as required per policy.
Maintains awareness of the health status of residents and facilitates transition to appropriate level of care/ service needs within Wesley continuum of care.
Demonstrates understanding of the continuum of care and service
offerings provided by Wesley on campus and in the community,
Monitors hospitalizations and ER visits,
Participates in resident review meetings,
Maintains contact with residents who experience a change in health status, new diagnosis, medication changes, etc. to provide support, education, and reassurance.
Demonstrates ability to accurately assess for appropriateness and eligibility for a change in level of care/service
Effectively communicates with residents and families
Participates in QAPI activities as assigned.
Identifies areas for performance improvement within scope of responsibility.
Demonstrates understanding of workflow and critical thinking related to problem- solving and performance improvement,
Demonstrates ability to foster cooperation and collaboration between lines of business within Wesley to achieve positive outcomes and satisfaction ratings.
Collaborates with external business partners and referral sources to promote positive outcomes and experience for residents and fellow employees and partners. Participates in marketing
activities as appropriate.
Other duties assigned by supervisor, campus executive director and Homecare director.
QUALIFICATIONS/REQUIREMENTS

Education Background

Licensure, Registration, Certification
Current Washington State RN license.
Maintain current CPR certification.
Special Training/Skills
Possess ability to respond appropriately and accurately in emergency situations.
Ability to make sound professional judgments within the scope of practice of license.
Work Experience
Must be a self-starter,possess ability to work independently, be well organized and be able to market Homecare and catered living services to the residents on campus.
Ability to communicate effectively with residents, families, and coworkers.
Ability to read and write English to accomplish record keeping responsibilities.
Able to work one weekend per month as needed.
Physical Demands
Ability to walk, climb stairs, bend, stoop to perform necessary tasks throughout the campus setting.
Ability to lift 35# in emergency situations for resident safety.
INFECTIOUS RISK CATEGORY

HIGH RISK: Likely to have frequent occupational exposure with patient’s blood or other body fluids, may require protective equipment to be worn during some aspects of direct patient care.

SOME (COVID-19) RISK: Possible occupational exposure to residents or clients with COVID-19. May require personal protective equipment (PPE) to be worn during some aspects or seasons of job.
