Salary: $90,000 – $101,000 / yr DOE

What You’ll Be Doing:
You will ensure the delivery of person-centered, individualized and clinically competent care in the Assisted Living and Memory Care facilities. You will coordinate and manage the overall care delivered to residents by directing and supervising care staff and ensuring that services are appropriate to meet residents’ needs in accordance with federal, state and local regulations.

How You’ll Do It: 
Coordinate nursing functions and tasks for residents with physicians and other healthcare providers. Delegate and assign appropriate tasks to assistive team members. Lead team members by ensuring accountability, communication and effective work.
Assess residents’ need for medical treatment and communicate with appropriate department or Primary Care Physician to report changes in status. Communicate with resident, healthcare representatives, Primary Care Physician, and/or transfer committee when necessary.
Provide nursing care to residents in accordance with physician’s orders by recognizing, observing and reporting symptoms/conditions of residents.
Follow the Licensed Nurse Scope of Practice outlined by the Board of Nursing in the state in which you work.
Ensure the nursing plan and resident care plan are developed and documented and that resident’s care needs are met. Develop and coordinate plan of care related to patient skin integrity and wounds.
Perform assessments/data gathering related to function and physical status of residents, change in condition, medication management, potential restraints, admissions, etc. Consult with care team to individualize care/Activities of Daily Living (ADL) plans and promote team focus on achieving/maintaining the resident’s highest level of functioning.
Oversee medication storage, administration, documentation, treatments and delegated tasks.
Maintain a physical, social, and psychological environment which will be conducive to best interest and welfare of residents.
Maintain open lines of communication with resident families/significant others and healthcare representatives. Participate in resident care conferences and transfer committee discussions.
Record timely, complete, accurate physician orders, ADLs, medication and treatment sheets, restorative and nursing activity sheets. Maintain 24hr Report and follow up on all changes in condition.
Develop policies and procedures to assure that care, health and safety needs, and state and federal rules and regulations are being met.
Attend mandatory training and in-services. Be aware of changes in health and safety regulations, employee policies, and emergency procedures.
Supervise employees, including scheduling, training, assigning work, providing guidance and performance management.
Motivate employees through mentoring, training and encouraging teamwork. Provide constructive feedback when identifying learning needs related to clinical performance, resident care and services.
Oversee and participate in Quality Assurance (QA) program by completing monthly reports and logs of QA trends and analysis. Assume leadership role in staff development, quality assurance and infection control.
Be constantly aware of safety and follow up on identified risks. Respond to emergency calls on campus per protocol. Complete incident reports and investigations for any accidents, medication errors, emergency situations, etc.
Support leadership in facilitating training of nursing personnel and supervising staff.
Observe universal precautions when providing resident care.
Perform first aid or other functions of Caregiver as needed.
Manage infection control program for residents and staff members.

About You:
Valid Registered Nurse (RN) license in the state of Washington.
Core Nurse Delegation Certification (Washington) is required.
Two (2) years’ professional nursing experience in Assisted Living or a long term care setting.
Current and valid CPR/AED and First Aid certification.
Prior supervisory experience in a similar setting including performance management, discipline, prgram development and management of staff.
Demonstrated ability to work independently and make sound judgements concerning all aspects patient care.
Excellent understanding, skills and ability to communicate patient care and sensitive issues.
Computer skills, including proficiency in Microsoft Office programs and medical records and clinical software programs.
