POSITION SUMMARY:

The MDS Manager is responsible for the timely, accurate, and comprehensive completion of the Resident Assessment Instrument (RAI). The incumbent will utilize an interdisciplinary team approach to achieve the highest resident outcomes. The MDS Coordinator/Restorative Manager oversees all aspects of the resident restorative program.

EDUCATION & EXPERIENCE:

1.Current RN license preferred
2.Three years’ experience in a long term care setting.
3.Three years’ experience in MDS
4.Current AED, CPR training required.

KNOWLEDGE, SKILLS & ABILITIES:

5.Knowledge of nursing practices and a demonstrated understanding of the MDS system.
6.Regular, dependable and reliable attendance.
7.Demonstrated supervision and leadership skills.

Essential Duties as MANAGER:

8.Completes all annual staff performance appraisal forms, directly or through subordinates; delivers to Human Resources through department head by due date. Meets with staff members on an ongoing and continual basis to review performance and provide feedback, guidance and instruction needed.
9.Selects candidates, trains, assigns and supervises staff, directly or through subordinates. Ongoing evaluation of performance of staff, initiates disciplinary action and consults with Human Resources regarding employee issues.
10.Plans for and recommends space, staffing, training and equipment needed to meet resident needs and /or to provide other services. Needs are demonstrated by budget, goals and objectives, reports and other departmental records.
11.Participates in applicable committees and meetings as required.
12.Completes required in-services.
13.Responds to internal and external communication timely and appropriately utilizing customer service approach.
14.Promotes a teamwork mentality throughout all aspects of position.
15.Communication and support of campus wide initiatives and goals to staff level to ensure all employees are aware of the
16.Develops & implements departmental policies & procedures.
17.Supports employee Safety programs and initiatives, ensuring no lost time caused by work related injuries.

ESSENTIAL FUNCTIONS:

18.Performs assessments, completes MDS for residents and transmits results to the state in accordance with reporting and reimbursement regulations.
19.Plans, develops and implements resident restorative programs in accordance with federal and state regulations.
20.Participates in the development, maintenance and implementation of the clinical quality assurance activities QAPI.
21.Participates in the resident care conferences as a member/leader of the interdisciplinary team.
22.Participates in Nurse Management tasks including sharing of on-call time.
23.Responsible for the Infection Control program, processes, and logs.
24.Audits medical forms, reports, evaluations, studies, charting, etc. for trends and learning opportunities.
25.Ensures timely response to incident reporting practices.
26.Collaborates with clinical staff to ensure a holistic approach to services is considered.
27.Meets with Charge Nurses to assist the team to interpret and plan daily needs of residents.
28.Collaborative treatment planning with therapists, nurses, NACs, and wellness assistants to assure the highest level of resident function.
29.Coordinates and leads weekly Medicare Review meetings.
30.Provides ongoing staff education on proper coding and ADL’s and resident function.
31.Participates in Triple Check meetings.

PHYSICAL DEMANDS:

32.Able to move intermittently throughout the workday.
33.Able to lift, push, pull and move a minimum of 50 pounds.

WORKING CONDITIONS:

34.Exposure to bloodborne pathogens:Level III.
35.Works in office area as well as throughout the nursing service area.
36.Subject to frequent interruptions.
37.Subject to hostile and emotionally upset residents, family members, personnel, and visitors
38.Works beyond normal working hours and on weekends and holidays when necessary.
39.Subject to falls, burns from equipment, odors, etc. throughout the workday.
