Hospice Social Work Supervisor $10,000 Sign on bonus

We are immediately hiring for a: Hospice Social Work and Spiritual Care Supervisor and includes a $10,000 Sign on Bonus

Responsible for supervising all aspects of the provision of patient social services and spiritual care. Establishes, implements, and evaluates goals and objectives for social services and spiritual care to meet and promote the standards of quality and contribute to the organizations mission and philosophy.

Essential Job Functions/Responsibilities

1. Oversees all daily patient social service and spiritual care operations including providing direct oversight of the establishment and implementation of social service and spiritual care policies.

2. Ensures compliance with all state, federal, and CHAP social service and spiritual care regulatory requirements.

3. Directs the implementation of improved work methods and procedures to ensure social service and spiritual cares are delivered in accordance with policy.

4. Assists the Hospice Operations Director in the preparation of an annual budget for the social service and spiritual care department and monitors allocation of resources according to budgetary limitations.

5. Participates in the recruitment, selection, hiring and orientation of the social service and spiritual care team.

6. Supervises the assessment, planning, implementation, and evaluation of social service and spiritual care clinical care by members of the social work team.

7. Assures proper documentation of social service and spiritual cares in clinical records in compliance with state, federal, and CHAP standards.

8. Provides leadership in strategic planning including identifying opportunities for additional or improved services to address unmet customer needs.

9. Maintains comprehensive working knowledge of the field of social work and shares information with appropriate organization personnel.

10. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Kline Galland Hospice Services.

11. Plans and implements in-service and continuing education programs to meet the development needs of social service and spiritual care personnel.

12. Actively participates in quality assessment performance improvement teams and activities.

13. Maintains a caseload and performs duties of hospice social worker.

Position Qualifications

1. A Master’s degree in Social Work.

2. At least three (3) years management experience preferably in home care operations.

3. Demonstrates an ability to supervise and direct professional and administrative personnel.

4. Ability to deal tactfully with customers and the community.

5. Knowledge of corporate business management, governmental regulations and
CHAP standards.

6. Demonstrates good communications and public relations skills.

7. Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities.
