Optum Complex Care Management, through the UnitedHealthcare Nursing Home and Assisted Living Plans, partners with skilled nursing and assisted living communities, providing added on site medical attention for our members. Advanced practice clinicians coordinate the care of residents at the bedside and support their families with frequent communication about their loved one’s care plan. With over 1,900 skilled nursing and assisted living communities serving over 60,000 members in 28 states, our clinical model has proven to: enhance quality of care, reduce unnecessary and avoidable emergency room visits and support census stability.
