Provider Relief Funds for Private Pay

We have received an update regarding the provider relief funds for private pay assisted living. The portal where providers can submit their data to obtain the relief funds is now live, and you can access that portal here.

As a reminder Department of Health and Human Services (HHS) is considering a possible distribution of CARES Act Provider Relief Funds to private-pay assisted living/memory care providers that are stand-alone or part of a CCRC. At this time, there is no guarantee this distribution will happen or any details on how and when funds may be allocated.  

Because there was a delay in the portal going live, HHS has extended the deadline to submit information to Monday August 3rd, at 5 pm ET. We are asking all eligible members to access the portal and submit their information as soon as possible. To those that have sent their info to David, you will need to resubmit through this portal. Contrary to what we were told, we are unable to submit this data on your behalf. You can find more details here. Portal Announcement   

For private pay, non-Medicare, or Medicaid contracted skilled nursing facilities and adult day programs LeadingAge National is collecting your information in hopes of a future allocation to your settings. Please submit the following information to Nicole Fallon at nfallon@LeadingAge.org.

What data must be submitted?

  • Name of licensed/registered/certified assisted living and/or memory care community
  • Name of entity that holds the license/registration/certification
  • Address
  • Community license/registration/certification number (some states may not provide a number)
  • Corresponding Tax Identification Number (TIN)
  • Taxpayer ID Number for the filing company
  • Contact information for the person submitting data
