The Background Check Central Unit (BCCU) published an updated BCCU Applicant Affidavit Form (DSHS 27-109) in November 2020.  Effective June 1, 2021, BCCU will no longer accept the previous BCCU Applicant Affidavit (REV. 04/2018).

Highlights of the November 2020 updates include the following:

• Check box for applicants to authorize BCCU to leave a detailed message at the phone number provided.
• Electronic signatures now allowed with attachment of valid photo identification.
• Update to the email address.
• Update to perjury and permission statement.
• Administrative updates to instructions to assist applicants in completing the form correctly.
