Please Help Us With a Memory Care Survey by July 14th

Washington Health Care Association and LeadingAge Washington have been working in collaboration with the Dementia Action Collaborative, and we are in need of your expertise and input regarding dementia care in assisted living facilities like yours.  We recommend a direct manager of the memory care unit/area/building associated with your facility’s license review this email and respond to the survey below.

Please complete the survey by July 14.  If you have questions regarding our work with DAC or the information we are requesting in this survey, please contact me or Alyssa directly; our contact information is at the end of this email. The following is a note from the DAC:

The Dementia Action Collaborative (DAC), a group of public-private partners guiding and implementing the Washington State Plan to Address Alzheimer’s Disease and Other Dementias, wants to understand what is meant by the terms “memory care” or “specialized dementia care” in residential care facilities. The DAC invites your input through an online survey to inform our work as we consider how to best help consumers get the care that responds best to their needs. As a provider doing this work, we are asking for your insights and perspectives. The brief survey is intended for Assisted Living Facilities and Adult Family Homes in Washington state that have identified themselves as providing “memory care” or “specialized dementia care” (either on a website or in some database).

Your participation in this survey is voluntary: your participation (or not) will have no impact your relationship with your provider association, the Dementia Action Collaborative or DSHS. Your name and the responses you provide are confidential and will not be connected with your facility name in any reports. The survey is relatively brief and will likely take no more than 15 minutes. We at Washington Health Care Association and LeadingAge Washington are partners in this work and would very much appreciate your input.
