This deadline applies to all Medicaid, CHIP, dental providers and those Medicare A providers who have not yet received any funds due to a Change in Ownership, and Medicare providers who have received funds but they have not yet reached 2% of gross patient service revenues as reported on line 9 of their form 990. This might also be an opportunity for providers who now have their 2019 990s completed to submit the updated information to obtain a potentially higher amount. Providers just need to complete the Tax Identification Number Validation step of the application process by 11:59p ET on Sunday, September 13th.  This is new info and is another extension of the deadline.
