Dear LeadingAge Washington Members:

We are excited to announce that through our partners, the Washington State Hospital Association, we have purchased and secured 5,000 surgical masks.  We are taking orders for masks using this purchase order.  We purchased these masks at cost, $.76 per mask, and are pleased to similarly provide them to you at cost.  A description of the surgical masks is included on the order form along with pick up or mailing details.

For all orders received by Monday, April 20th, you may take delivery on Tuesday, April 21st at one of the three convenient locations offered – Everett, Southcenter, and Tacoma.  Alternative pick up arrangements may be made by calling the person assigned to the region closest to you.  We are also able to ship the masks if you prefer and plan to ship any orders received from our members east of the mountain. 

Please order the number of masks you believe are needed.  Although we may not be able to fulfill your order completely, we will rely on the number of masks requested as an indicator of the number needed when we place subsequent orders.
