This session will delve into recent changes to MDS-based frozen QMs, providing clarity on adjustments that may impact scores in the January 2025 refresh.
Participants will also learn actionable steps to address rating triggers and calculate QM points, enabling proactive management of ratings and payments, and explore the specific measures impacting Washington’s QE payments.
Bring your most recent 5-Star and your Quarter 3 (7/1 – 9/30) Facility-level and Resident-level CASPER reports, then join us to deepen your expertise in the systems that drive quality and reimbursement, ensuring your facility continues to excel in delivering exceptional care.
Objectives:
- Understand which Quality Measures are MDS-driven vs claims-based and what that means for your 5-Star points.
- Review of 5 MDS-based frozen QMs: new definitions, and how to determine if you will be losing or gaining 5-Star points in January.
- Better understand what you can do now prior to the January refresh, including how to identify individual triggers that may be incorrect or negatively impacting your rating.
- Discussion on Quality Enhancement payment: which measures are impacting this, how to determine upcoming payment(s), and what you can do now.
- How to calculate your points.

Andrea O’Neill, President of Cornerstone Clinical Consulting
Andrea has been the President of Cornerstone Clinical Consulting since April 2023.
Prior to this, Andrea spent time in various SNF roles, from NAC in 2002 to Unit nurse in 2006, MDS from 2008-2017, then DNS and Regional Clinical Director from 2017-2023.
While a DNS, Andrea guided her MDS team in MDS / RAI education to develop systems to support successful increases in cmi, achieve the top tier in WA Quality Enhancement (QE) rating, and a 5-Star rating overall including 5-Star Quality Measures. In her regional role, Andrea oversaw multiple facilities to support compliance with federal regulations, survey readiness, completing or helping with plans of corrections, infection control compliance as well as ongoing training for nurse leaders to better understand the MDS and QM/QE systems.
Andrea has efficiently trained MDS nurses with no experience or case mix knowledge to improve and maintain entitled credit, with significant increases in facility reimbursement. While Cornerstone primarily consults for MDS reimbursement, given Andrea’s long-standing and wide-range of skilled nursing experience, she also works to assist facilities she works with to achieve and maintain 5-Star QM’s and increase their overall star-rating.
