Sea Mar’s Cannon House assisted living facility is located in Seattle’s Central District. The mission of Cannon House is to provide safe, affordable, and high-quality assisted living for retirees, seniors, and others in need of residential care who want to live as independently as possible. The Wellness Coordinator will coordinate, manage and supervise Resident care as assigned.  This includes the responsibility of implementing and ensuring completion of systems of care. The Wellness Coordinator must demonstrate knowledge and skills necessary to provide care appropriate to the age of the residents served, knowledge of the principles of growth and development over the life span, and be able to identify and work to meet each resident’s age-specific needs.

Job Description/Essential Duties:

Under the direction of the Resident Care Manager the Wellness Coordinator will:

  • Assist the Resident Care Manager in the coordination, management and supervision of Resident care
  • Assist the Director of Nursing Services / Resident Care Manager in the supervision of nursing staff as assigned (24 hour accountability).
  • Insure Resident care plans are current, accurate and complete and based on a thorough assessment of the Resident’s status and care needs. The Wellness Coordinator will involve the Resident in the development of the Care Plan. 
  • Initiate and attend resident care conferences as needed
  • Address family and patient complaints and arrive at a viable resolution.
  • Conduct resident assessments such as Fall Risk, Skin at Risk, Psychotropic medication evaluations and others for residents, as necessary.
  • Round with staff as needed
  • Note physician orders
  • Receive and manage relevant community telephone calls.
  • Order medications from pharmacy.
  • Ensure proper and timely documentation relative to resident services.
  • Attend departmental meetings as assigned.
  • Maintain an emergency crash cart, as needed
  • Review admission orders and admission assessments for completion and accuracy.
  • Appropriately delegate duties to nursing department staff
  • Delegate assignments with awareness of qualifications necessary for completion.
  • Implement established protocols when resident condition indicates.
  • Administer and evaluate resident response to medications as necessary.
  • Assure compliance with all resident treatment orders.
  • Formulate and implement nursing plan of care and orders.
  • Evaluate Resident response to nursing intervention for continuance and/or revisions.
  • Coordinate multidisciplinary plan of care.
  • Assure narcotic count log is current and accurate.
  • Assure that charting demonstrates resident condition and is reflective of the nursing care provided.
  • Ensure that incident reports are completed and reported per state regulations.
  • Demonstrate technical nursing skills.
  • Assist RCM with staff recruitment, interviews, training, etc. as needed..
  • Assists RCM with yearly performance evaluations for staff.
  • Counsels/reprimands/terminates personnel as per organizational policies and procedures and Union contract.
  • Participate in Quality Improvement and Joint Commission activities as needed
  • Participates in organizational/community events and activities.
  • Performs other duties as assigned.

Required Education/Training/Experience or Special Skills:

  • Licensed Practical Nurse or Registered Nurse
  • Current professional licensure in Washington
  • Current CPR certification

Additional Information:

  • LPN with 1 or more years of experience in Long Term Care/Assisted Living preferred
  • Current professional licensure in Washington
  • Recognition of ANA Standards, Scope of Gerontological Nursing Process in Practice
  • Evidence of Supervisory and Leadership skills
