Tacoma Lutheran Retirement Community is seeking a Social Worker for our Skilled Nursing facility!

Full-Time, Monday-Friday Schedule

Summary
Perform a variety of services offered by the Social Services Department, which will enhance the psycho-social-emotional, and intellectual development of the residents of Tacoma Lutheran Home and their families with personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.
Qualifications
EDUCATION and/or EXPERIENCE
• Bachelor’s degree (B. A.) from four-year college or university;
• or one to two years’ related experience and/or training;
• or equivalent combination of education and experience.

LANGUAGE SKILLS
• Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
• Ability to write routine reports and correspondence.
• Ability to speak effectively before groups of customers or employees of organization.

REASONING ABILITY
• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
• Ability to deal with problems involving several concrete variables in standardized situations.

Physical Demands
• Regularly required to sit; use hands to finger, handle, feel, write or type; and talk or hear.
• Occasionally required to reach with hands and arms.
• Must regularly lift and/or move up to 10 pounds.
• Specific vision abilities required by this job include close vision, and ability to adjust focus.

Essential Duties and Responsibilities
• Will provide direct and regular services including:
1. Resident and family counseling
2. Crisis intervention and community referral.
3. Program development and implementation
4. Participation in interdisciplinary team conference.
5. Maintenance of documentation as required by state and federal regulations.
6. Utilization of community resources.
• Serve as advisor to the Resident Council for assigned area.
• Shall participate in New Family Orientation.
• Shall participate in New Resident orientation
• Shall participate in Family Council.
• Will be responsible for intra-facility transfers with the Admission Coordinator.
• Will attend networking and educational meetings as determined by the Director of Resident Services.
• Responsible for developing awareness of community resources.
• Will be involved with inservice education in the department, facility, and the community.
• Shall utilize the services of a MSW Consultant through private consultation and educational opportunities.
• Will advocate for and protect the rights of residents.
• Prepares patient histories, service plans, and reports.
• Participates in planning for improving health services by interpreting social factors pertinent to development of program.
• Provides general direction and supervision to workers engaged in clinic home service program activities.
