POSITION SUMMARY:

The Social Services Coordinator is responsible for managing programs that meet the psycho-social needs of residents residing in Penrose Harbor. The coordinator plans, organizes, develops Social Services operations in accordance with state and federal regulations and guidelines.

EDUCATION & EXPERIENCE:

  • Bachelor’s degree in social services or related field
  • Three years’ experience in a long term, geriatric care setting.

KNOWLEDGE, SKILLS & ABILITIES:

  • Knowledge of federal, state and local regulations.
  • Initiative and creativity.
  • Skilled in recognizing physical, mental and emotional needs of residents and good judgment in making decisions about actions to be taken regarding those needs.
  • Excellent inter-personal skills.
  • Tactful, enthusiastic and courteous when interacting with residents, family members and staff.
  • Empathy and sensitivity to needs of ill, aged and disabled persons.
  • Patience, poise and dignity in an emergency or crisis.
  • Ability to work in close cooperation with professional and non-professional staff members
  • Ability to maintain confidentiality.
  • Excellent English communications skills both verbal and written

Essential Duties as MANAGER:

  • Completes all annual staff performance appraisal forms, directly or through subordinates; delivers to Human Resources through department head by due date. Meets with staff members on an ongoing and continual basis to review performance and provide feedback, guidance and instruction needed.
  • Selects candidates, trains, assigns and supervises staff, directly or through subordinates. Ongoing evaluation of performance of staff, initiates disciplinary action and consults with Human Resources regarding employee issues.
  • Plans for and recommends space, staffing, training and equipment needed to meet resident needs and /or to provide other services. Needs are demonstrated by budget, goals and objectives, reports and other departmental records.
  • Prepares annual budget to provide appropriate level of care and establish financial goals. Justifies assumptions used in compilation.
  • Participates in applicable committees and meetings as required.
  • Completes required in-services.
  • Responds to internal and external communication timely and appropriately utilizing customer service approach.
  • Promotes a teamwork mentality throughout all aspects of position.
  • Communication and support of campus wide initiatives and goals to staff level to ensure all employees are aware of the organization’s overall goals and objectives.
  • Develops & implements departmental policies & procedures.
  • Supports employee Safety programs and initiatives, ensuring no lost time caused by work related injuries.

ESSENTIAL FUNCTIONS:

  • Completes supportive visits to residents as indicated in the plan of care and as needed to meet psychosocial needs.
  • Collaborates with clinical staff to ensure a holistic approach to services is considered.
  • Obtains and documents in residents’ medical chart their comprehensive social history.
  • Establishes plan of care for each resident within regulated time lines.
  • Documents resident’s progress in the resident’s medical record.
  • Conducts discharge planning and coordinates resident-directed discharge plan goals.
  • Assists with investigations of incidents related to resident’s psychosocial wellbeing including allegations of abuse.
  • Assists with investigation of lost or stolen resident property.
  • Provides opportunity for resident/family to participate in plan of care i.e. care plan meetings, care guide updates/revisions.
  • Utilizes appropriate tools for resident assessment.
  • Manages PASRR screenings according to state regulation.
  • Informs/assists residents understand their rights to create and execute advanced directives.
  • Maintains resident feedback form/grievances process and state required log.
  • Prepares and delivers resident communications i.e. Medicare and room change notices.
  • Coordinates return of resident’s possessions to the family/responsible party after discharge.
  • Maintains resident and family educational informational files and provide/implement when appropriate i.e. resident rights, community resources, facility agreements and contracts, Medicare/ Medicaid, interpretive services, transportation, hospice services, mental health services.
  • Be observant of changes in residents and report immediately to nursing staff.
  • Conducts initial, quarterly and annual assessments as appropriate.
  • Collaborates with the MDS Coordinator to ensure accurate and timely MDS completion.
  • Implements and maintains Quality Assurance Program for the Social Services departments i.e. psych rounds, resident feedback form patterns.
  • Provides in-service/orientation training to staff involving social services topics i.e. resident rights, abuse and neglect.
