RN MDS Case Manager

Conduct a comprehensive admission assessment for new patients.
Initiate an individualized nursing care plan based on assessment data, and shares important information about patients with IDT members.
Oversees Resident Assessment Instrument (RAI)/MDS process for assigned patients, including: monitoring the assessment schedule in collaboration with Health Information staff; timely and accurate completion of Minimum Data Set (MDS) nursing assessments; and coordination of completion of other MDS sections with IDT members.
Updates care plans regularly in collaboration with the IDT.
Works closely with team members to develop and implement a nursing discharge plan.
