JOB SUMMARY

Provides direct care and case management for a team of WHAH patients in accordance with the state Nurse Practice Act and WHAH policy. Directs assigned team members of RNs, LPNs, and Home Health Aides.

ESSENTIAL FUNCTIONS

Complies with accepted ethical conduct and professional Standards of Nursing. Practice set forth by the American Nurses Association.
Assesses client’s condition and needs through the collection of health data and the recognition, interpretation and reporting of symptoms and psychological responses; re-evaluates the client’s nursing needs as required.
Develops and implements a nursing plan of care with consideration for the dignity and desires of the client, organizes and participates in direct client care, performs treatment and administers medications while providing direct patient care.
Formulates, modifies and carries out a client care plan which prescribes nursing actions toward achievable goals, according to the client needs.
Initiates and participates in the client admissions process and in the development of the Plan of Treatment, obtains Physician’s orders, clarifies those orders and obtains necessary revisions as needed, informs the Physician of changes in the client’s condition and provides written summary reports to the Physician.
Other duties as assigned (Participates in health teaching and training with client and/or family members; maintains detailed records of nursing actions, client progress and response to therapy and teaching; coordinates discharge planning and prepares discharge summaries and instructions; coordinates Nursing, HH Aide, PT and other services delivered to clients; etc.)
QUALIFICATIONS/REQUIREMENTS

Education Background

Graduate of an accredited school of professional nursing.
Licensure, Registration, Certification
Current license as a Registered Nurse in Washington State. Baccalaureate degree preferred.
A current and valid Washington State driver’s license and valid vehicle insurance.
Current CPR certificate
Special Training/Skills
Ability to supervise and direct members of patient care team.
Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
Ability to deal tactfully with patients, providers, and families, and the community.
Demonstrated organizational and time management skills.
Knowledge of home health regulatory and reimbursement requirements.
Work Experience
Minimum of two (2) years of nursing experience. Home health experience preferred.
Minimum of one (1) year supervising LPN’s, NAC’s or Healthcare Staff.
Physical Demands
Ability to stand/walk 2-3 hours per day
Ability to bend, stoop and squat frequently for resident care and assessment
Ability to sit for periods of time for assisting residents with meals, charting, phone consulting, and computer data entry.
