Oversees and manages the daily operations of the ICHS Program for all Inclusive Care for the Elderly (PACE) including operations of the day center, home care services, PACE clinic, social work, rehabilitation, recreation, spiritual care and dietary. This includes ensuring that appropriate personnel are trained and perform their functions within the organization, operational services standards and budgetary criteria are met, and operations are in compliance with all governing regulations.

Core Responsibilities:
Oversee the daily coordination and the implementation of the Program for all Inclusive Care for the Elderly Model of Care.
Continuously evaluate the participants’ changing needs and makes necessary program adjustments.
Assist in providing in-service education for the Program for all Inclusive Care for the Elderly staff.
Direct, supervise, and evaluate the performance of the Day Center Supervisor, Speech Therapist, Recreational Coordinator, Social Worker, Dietitian, and Receptionist.
Ensure continuity of operations with all policies and procedures. Ensure operational processes are in place.
Assume the Program Director’s responsibilities in her/his absence.
Ensure that the job responsibilities, authorities and accountability of all direct reports are defined and understood.
In conjunction with the Program Director, responsible for maintaining compliance with all state and federal program requirements, obtaining and maintaining certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.
Assist in the development and implementation of the organization’s Program for all Inclusive Care for the Elderly Strategic Plan.
Assist in program development and preparation of the Program for all Inclusive Care for the Elderly’s short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives.
Participate in all interdisciplinary team meetings and serves as facilitator.
Coordinate the interdisciplinary team’s care management process to ensure best decisions are made regarding the participant’s changing condition.
Oversee and direct the coordination of home health, rehabilitative, long-term care and medical/specialty services according to participant care plans.
Assist Program Director in development of annual operational and capital budgets for departments of accountability and managing services to meet financial benchmarks.
Assist Human Resources Generalist in development and implementation of Program for all Inclusive Care for the Elderly operational policies, procedures, protocols and regulations for direct participant care and support services.
Monitor operations to ensure consistent compliance with established quality assurance standards.
Assist Program Director with evaluation of the Program for all Inclusive Care for the Elderly performance against national results and established service standards, budget and other financial criteria.
Assist in development and implementation of performance improvement plans based on Quality Assessment findings.
Recommend changes in service delivery and staffing as required due to the development and growth of the organization.
Participate regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
In conjunction with the Program Director and the Quality Assurance Director, maintains the stability and reputation of the Program for all Inclusive Care for the Elderly by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing operations.
Establishes positive working relationships with health and social service providers.
Protect privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.
Participate in continuing education classes and any required staff and training programs.
Maintain professional affiliations and any required certifications.
Provide administrative on-call coverage on a rotation basis.
Practices standard precautions and follows appropriate Infection Control procedures.

Position Requirements: Education – Bachelor’s Degree in business administration, or a health or human services field, or Registered Nursing degree/diploma required. Professional licensure and Master’s Degree preferred.

Experience: A minimum of five years of health care experience, and three years of management experience, preferably in a geriatric care setting preferred. A minimum of one year’s experience working with the frail or elderly population is required. Either training or related experience in direction of a health care program. Knowledge of the physical, mental, and social needs of the elderly.
