The Health Services Coordinator assists the Wellness Manager in ensuring that residents receive high quality personal and health services identified in their negotiated service plans; addressing resident issues and concerns regarding personal care; updating annually personal care portion of resident assessment/negotiated service plan for each resident; updating personal care services as needed; assisting with health monitoring and medication assistance program; providing guidance to resident assistants as needed to ensure smooth work flow operations and high quality resident care; and providing resident assistant training as needed.

Essential Duties: Include the following; other duties may be assigned depending upon project, client or workflow needs.

  • Addresses resident issues/concerns regarding personal and health care services by investigating problems and identifying solutions with resident and staff.
  • Updates weekly resident task list, personal care portion of resident negotiated service plan, and resident flow record as needed.
  • Communicates changes in personal care plan to resident assistants and Wellness Manager.
  • Coordinates personal and health care with Wellness Manager to ensure high quality of resident care.
  • Annually updates personal care portion of resident assessment/negotiated service plan in a timely manner.
  • Participates in resident care conferences.
  • Assists in staff training related to medication assistance, personal care services, and blood glucose testing.
  • Assists the nurse to promote clients and resident independence by educating about their disease process, diet, and procedures for health monitoring.
  • Provides specific care direction for Resident Assistants in observing the residents’ and clients’ condition.
  • Provides medication assistance to residents as needed.
  • Coordinates resident medical appointments.
  • Documents residents’ changes of condition, attendance at medical appointments.
  • Contributes to team effort and organizational goals through respectful communication, great customer service, and staying involved in organizational changes.
  • Meets all agency and state training requirements and enhances own professional knowledge through continuing education offerings.
  • Adheres to agency organizational values, standards, policies and procedures.

Required Education/Training/Experience or Special Skills:

Qualifications:

  • Team player and good interpersonal and communication skills
  • Ability to provide services in multi-cultural setting
  • Bilingual ability in Asian languages desirable
  • Has experience working with dementia population
  • Has professional experience with health related teaching

Knowledge, Skills, and Abilities

  • Knowledge of health care systems
  • Ability to communicate with dementia clients/residents
  • Demonstrated experience in health related teaching
  • Team player and good interpersonal and communication skills

Physical Demands/Working Conditions:

To perform these duties, the employee is required to stand, walk, in occasion perform a one-person transfer, and sit and use a keyboard.

Education & Experience: Any combination of experience and training that would likely provide the required knowledge, skills and abilities will be considered.

  • Two years or more of post-secondary (college) education plus technical training in related field.

Licenses or Certificates:

  • Completed Fundamental of Care
  • First Aid/CPR
  • Food Handler permits

  • Work is performed in an office environment with a low noise volume. While performing the duties of this job, the employee is frequently required to read, speak, stand, walk, sit and use a computer keyboard and monitor.
